If you lose your pet, call CHS at 262-554-6699 to make a Lost Animal Report. We will keep this information on file and contact you if an animal matching your description arrives here at our Shelter. Then follow-up your call with regular visits to our Shelter to see if your pet is here. When you have identified your pet, there are a couple of things you need to know before reclaiming your dog or cat.

You will need to bring the following five (5) things with you:

1. Proof of currently valid rabies vaccination.
   Proof is the paperwork certificate from your vet; a rabies tag is not proof. If you do not have your paperwork, you will need to go to your vet clinic and pre-pay for a rabies vaccination. Bring this new paperwork with you. You may also pre-pay for a rabies vaccination at CHS. You will then need to return to us, with the rabies vaccination paperwork, after you obtain the vaccination, so that your license can be processed.
    
2. Proof of currently valid license (where applicable in your municipality).
   Proof is the paperwork you received when you purchased your license; a license tag is not proof. If you do not have this paperwork, you may purchase your license here at our Shelter at the time of reclaim, for the fee established by your municipality plus a $5.00 Counter Fee. Proof of rabies vaccination is necessary for licensing.
    
3. Your driver's license or state photo I.D.
    
4. Reclaim fees to be paid in cash, or by VISA or MasterCard. Reclaim fees are variable and will be discussed with you when you identify your animal.
    
5. Leash and collar for your dog, or carrier for your cat.
